Compliance Forms for Student-Athletes

Sports Medicine

Prior to participation on a Tufts University varsity athletic team, student-athletes must submit required emergency and health history documentation to the Tufts Sports Medicine Department. In order to do so, all student-athletes will be asked to create an account via SportsWareOnLine. Instructions for creating your SportsWareOnLine account can be found HERE (PDF).

OEO Incident Reporting

Report discrimination and harassment, including racial and sexual misconduct, anonymously at Tufts-OEO.ethicspoint.com

Ethicspoint is a third-party secure web tool and allows any member of the Tufts community to file a report or complaint based on race, color, national origin, ethnicity, religion, sex, gender, sexual orientation, gender identity and expression (including a transgender and/or gender non-conforming identity), disability, retaliation for filing a prior complaint and other identity issues. No issue is too small to report. Our goal is to stop future conduct of this type not to focus on who reported it.

EADA Reporting

The Tufts University Athletic Department submits its Equity in Athletics Disclosure Act (EADA) report to the U.S. Department of Education's Office of Federal Student Aid annually. The Equity in Athletics Disclosure Act requires co-educational institutions of postsecondary education that participate in a Title IV, federal student financial assistance program, and have an intercollegiate athletic program, to prepare an annual report to the Department of Education on athletic participation, staffing, and revenues and expenses, by men's and women's teams. The Department uses this information in preparing its required report to the Congress on gender equity in intercollegiate athletics.  Tufts' EADA report can be accessed online via the U.S. Department of Education website: Equity in Athletics Data Analysis

The 2023-24 Tufts University Report on Athletic Program Participation Rates and Financial Support Data can be obtained upon request from the Department of Athletics by emailing athletics.admin@tufts.edu
